April 2012 Activity Report
Operations and Administration
. There were 18 total calls in April.
. 8 Fire related.
. 11 EMS related
. 0 Dive related.
. FD spend is consistent with planned budget.
. The new CAD/RMS system is moving forward, still waiting for the county to
confirm the hardware that we will need to purchase for this system.
. The state has approved Lakeview's license request. We can now start moving
forward with permanent accommodations.
. Annual Lions Chicken Dinner went good, had a good turn out.
. The school bus class went very good. Everyone felt that they gained knowledge
from this class.
. We tested 6 recruits and would like to have all 6 of them start June 2�d
. Lakeview management was present at our rescue drill to let everyone know how
the license transfer was proceeding and to answer questions or concerns that
any members had.
. We are looking at possible options for starting to replace our turnout gear. Most
of our gear is older than the NFPA standard of 10 years of service and it is
beginning to show.

. Fire Training: Low angle rescue techniques.
School bus extrication class hands on portion.
. Rescue Training: Lakeview Ambulance provided training for us; we covered
setting up an IV & how to operate there new electric stretcher.
. Dive training: Equipment refresher & boat opps.

This letter is to notify you the ambulance service—primary service area (PSA) changes that have
been requested by Lakeview EMS —EMSRB license#0242 & Lakeview EMS —EMSRB license
#0150 have been approved. The changes are in accordance with Minnesota Statutes, section
144E.07.
I am enclosing the Minnesota Emergency Medical Services Regulatory Board's approval
decision—BOARD ORDER PSA-2012-14-EMSRB. I have also enclosed the written PSA
descriptions that will now be the licensed PSA's for each of the services.
With the approval process now completed, it will now be necessary to compare and adjust the
primary service area that you have on file in your 911 system to reflect the changes approved for
Lakeview EMS.

areas(PSAs). Upon submission of a complete application by onc or more licenscd ambulance services
demonstratin�,com��liance with the c�•iteria specified in this stalute,the i�MSRB niay ap�xove the proposed
PSA changes withoufi requirin�a contested case hcaring.

III. Stafi'Commcnt
Staff review of ihe application affirms c��ite;ria specificd in Minnesoia Siaiutes §144I:.07 have been met.
The proposed PSA for each of the affected licenscd ambulance services are attacfied to this LMSR[3 order.
Revised PSAs will be forwardcd to each paity and to the Public Safety Answering Point(PSAP) upon
approval of this request.
Through thc PSA suenma�y approval process, Lakcview i�:MS will eliminate current PSA overlaps with license
�Q150 and will incorporate PS/1 under liccnse��O15U into Lakevicw EMS license�l 0242. "1'l�is summary
apk�roval will close license# O1 S0. Areas retracted in license#O1 SO will continue to be served by].,akeview
LMS license#0242.
The closi�ig of l,akeview EMS- Marine On St. C��oix license#0150 and the ex��ansion of Lakeview EMS
license#0242 will exceed the maximum times and distances identified in Minncsota Rules 4690.3400,subpart
3. I,akeview F�MS -#0242 has indicated in the summary approval a�plication that tiiey will maintain a staffed
substation at ]5041 Scandi�'I'rail Norih, Scandia. Maintaining the staffed substa#ion is a condition of this
o�•der. With the substation in place, I..akeview I.;MS��0242 will be in compliance with the maximum timcs
and distances for PSA in the afo��eme�itioned Minnesota Rule.
7'his non-c;ont��oversial I'SA change request will ensure continued EMS responsc to all areas of Washington
County and will benefit t1�e public living there.
Staff recon�mends approval is granted to l.,akeview �MS—Marine on St. Croix and Lakeview GMS -
Stillwater for the requested PSA changes.

PLEASL TAKE NOTICE that the Emergency Medical Services Regulatory Board(hereinafter EMSRB) has
received a completed application from North Memorial Ambulance Service,Forest Lake,Minnesota, for
expansion of their designated primary service area.
NOTICL IS HEREBY GIVEN that, pursuant to Minnesota Statutes § 144E.11, subdivision 3, each
municipalit��, county, community health board, governing body of a regional emergency medical services system,
ambulance :;ervice and other person wishing to make recommendations concerning the disposition of the
application, shall make written recommendations or comments opposing the application to the EMSRB within 30
davs or bv,June 7,2012, 4:30 p.m.
Written recommendations or comments opposing the application should be sent to: Pamela Biladeau, Executive
Director, EMSRB, 2829 University Avenue S.E., Suite 310, Minneapolis, Minnesota 55414-3222. If no more than
five comments opposing the application are received during the comment period, and the EMSRB approves the
application, the applicant will be exempt from a contested case hearing, pursuant to Minnesota Statutes § 144E.1 l,
subdivision 4(a). If more than five comments in opposition to the application are received during the comment
period, or the EMSRB denies the application,the applicant may immediately request a contested case hearing, or
may try to rf;solve the objections of the public and/or the EMSRB within 30 days, pursuant to Minnesota Statutes §
144E.1 1, sul�division 5(a), (b). If the applicant is unable to resolve the objections within 30 days, or if the applicant
initially reqtiests a contested case hearing, one will be scheduled and notice of the hearing given pursuant to
Minnesota Statutes § 144E.11, subdivision 5(c)-(e).
