MSHA I.D. # , Commercial Asphalt Co. plant # , other facility SAFETY PROCEDURES The following rules are provided for infrequent visitors, vendors, drivers, and short-term contractors to ensure that such persons are aware of the potential mining and plant safety and health hazards at this location. 1. "Think Safety" and always wear your seatbelt in your vehicle, truck, or piece of equipment. 2. Obey all traffic controls, speed limits (Speed Limit is 15 MPH), and safety awareness signs. Yield the right -of way to all off-road mobile equipment and never pass haul trucks. Come to a complete stop at all stop signs. Turn off vehicle and set parking brake if you leave your vehicle unattended. Turns wheels into a bank or berm, or use chocks, when parking on a grade or slope. 3. Personal protective equipment (PPE), including hard-hats, protective footwear, hi -visibility reflective vests and eye protection, are required on mine and plant property. Additional PPE such as fall arrest devices, full face protection, and hearing and respiratory protection are required during applicable safety and health exposure situations. 4. Visit only those areas that you are specifically authorized to visit and use only travel routes designated by mine or plant management, unless you are escorted by an experienced Tiller Corporation employee. 5. In the event of an emergency, go to the nearest pit or plant office or job site trailer for emergency call numbers located in the Emergency Action Plan for that site. This includes spills of hazardous materials including gasoline, diesel fuel, asphalt cement, etc. 6. In the event of causing or observing a release of hazardous materials, report the incident to the pit or plant foreman immediately so that the situation can be remedied as quickly as possible. 7. On mine property you must have an MSHA ID Number to self load any product and this number must be on file in our office. 8. Drivers and passengers MUST always stay in their cab while waiting to be loaded. Children must stay in the vehicle at all times. 9. Keep truck boxes and equipment at least 10 feet from overhead power lines. 10. Proper lock -out procedures will be followed when working on equipment. 11. Trucks must be tarped and completely sealed. 12. No jake brakes. 13. Trucks shall not idle on the site or on the approach area for more than 30 minutes. 14. Haul truck shall utilize a circular traffic pattern to minimize the need for trucks to back up on site. 15. Comply with instructions or warnings issued by Tiller Corporation employees, and report to pit or plant foreman any condition or behavior that you observe and believe to be hazardous. 16. I understand that violation of any of these rules could result in loss of my privileges to enter any Tiller corporation property. I understand that failure to follow these rules may cause injury or death to myself and others.
